With Dr Yoder’s retirement August 25, 2021 the Prescott, AZ office was vacated and no services or records related to Dr Yoder’s practice are available there. Patients were pre-notified by mail sent to all patients of record June 6, 2021.

 

The practice is closed and if medical record transfers are needed, please follow the instructions below.

Medical record requests: need to be in writing and signed by the patient or their legal representative, and dated. These requests are best sent from your new primary care provider only if they require these, so it is assured they desire and agree to receive them.

 

Mail requests to: 904 N. Overlook Circle  Payson, AZ 85541  Allow reasonable time for transfer as processing may be less than weekly and these records become more out of date and less relevant to ongoing medical care.
